Need to Update Health Records?
Updated records may be submitted through:
- email records to nurse
- sent in with student and dropped off in the nurse's office
- dropped off at the school during school hours.
Communication
It is the responsibility of the parent/guardian to tell the school nurse about any health concerns or chronic health issues that your student may have. Please be sure to complete the returning student registration form each year for your student. If your student's medical status changes during the school year, please contact the nurse.
Emergency Action Plans
Per school district policy, if your child has asthma, severe allergies, diabetes, or a seizure disorder, an emergency action plan is required to be on file with the school nurse and updated each year. EAPs are completed by the student's doctor. Emergency medications must be provided to the school as ordered by the doctor.
