Beginning with the 2025–2026 school year, Liberty Public Schools Athletics will transition from Privit to FinalForms as our new platform for all MSHSAA-required forms, emergency information, and physical documentation. You can get started with the setup process now for the upcoming year.
We’re excited to partner with FinalForms, an online forms and data management system that makes it easier for families to complete and sign athletic participation forms. One of the biggest benefits is that FinalForms saves your information from season to season and year to year, so you won’t need to re-enter the same details each time. The platform also pre-populates student information whenever possible, helping streamline the process and save time.

Q. When should I update my athletes information?
A. As soon as possible! Do not wait until that last minute. As we draw closer to tryout dates the system runs extremely slow due to multiple schools clearing. If you do not update your student at least 2-3 weeks before tryouts we can't guarantee they will be cleared in time and may be sitting out until we can get them cleared.
Q. Is my student cleared?
A. Please log in to your FinalForms account to view this information. Upon logging in you will see documents highlighted in green, which indicates they are complete. Items highlighted in red, indicate they are incomplete. Once all items are highlighted green, the student-athlete is cleared.
Q. Why can't my student's coach see them in FinalForms?
A. Coaches will only be able to see the athletes who have signed up for their particular sport. You must add your athlete to the sports they are interested in participating in.
Q. Can I send my paper physical with my athlete to tryouts?
A. No! Liberty Public Schools Athletic Departments will not collect any medical documentation. All required documents must be completed through FinalForms prior to tryouts. Coaches have been instructed not to collect any medical paper documentation. Parents and students are responsible for their paper physicals and uploading them to FinalForms.
Q. How long is my physical good for?
A. Per MSHSAA regulations, physicals are valid for two years unless otherwise specified by the examining physician. If the doctor determines a shorter validity period is necessary, such as one year, that will be noted on the physical.
